Facilities Use Request

Winlock School District now partners with FMX to provide an online Facilities Use Request Form. If you are a community member and/or group that would like to request the use of school facilities, please navigate to FMX. Use of facilities by school personnel, student-connected activities, and community organizations and groups is encouraged. The use of school facilities for educational and recreational purposes, public meetings, entertainment, and political meetings by outside agencies is permitted. Please note: The use of buildings and facilities to fulfill educational objectives of the school shall have preference over all other requests. Any facilities use request made by an out-of-district entity, organization, or person cannot be guaranteed between the hours of 7:30 am and 6:00 pm, Monday through Friday.

Persons and organizations requesting to use Winlock School District facilities must provide a Certificate of Liability Insurance, IRS Letter of Determination (proof of non-profit designation), a completed W-9, if applicable, and a volunteer/background clearance check before rental requests can be submitted.

Facilities Use Request FAQ

When requesting to use our facilities for an upcoming event, the process begins with our online FMX scheduling request system. This system assists the District Office when processing Facilities Use Requests. A request must be submitted at least 2 weeks prior to an event.

Please note: The Facilities Use Form is considered a legal-binding document and should be signed only by individuals aged 21 and over. Persons and organizations requesting to use Winlock School District facilities must provide applicable Certificate of Liability Insurance, IRS Letter of Determination (proof of non-profit designation), completed W-9, and volunteer/background clearance before rental requests can be submitted.

Please do not call the District Office and ask for immediate verbal verification, availability, or confirmation. Requests will be processed in the order in which they are received. Once registration is complete and all required documents are received and validated a facilities request can be placed. After you have submitted your request, your request will be routed to a series of approvers within Winlock School District. Once your request has been approved, you will receive an email notification. You will also receive an emailed copy of your invoice, if applicable. You can track your pending requests anytime by visiting https://winlock.gofmx.com/scheduling/requests.

Obtaining District Access Keys: After your request is approved you may pick up an access key from the office where the requested facility is. For example, if the Winlock Miller Elementary School Gym is requested, you will complete the key check-out form at the Winlock Miller Elementary School office. Only those that have completed volunteer/background clearance check will be given access/use of keys. Use of keys by unauthorized personnel will result in loss of access to the facilities. A lost or stolen key must be reported as soon as possible. There is a $10.00 fee for any key that must be replaced. All keys must be turned in to the office that issued the key no later than the end of the next business day.
